Student Information on Medical Withdrawal
Students experiencing significant psychological problems may be able to obtain a medical withdrawal from their entire course load. You will receive grades of WP or WF for all of your classes, but your overall GPA will not be affected. This withdrawal must be obtained by the day prior to the last day of classes of any given quarter. This medical withdrawal will result in a “medical hold” on your student record.
Please note that while the "medical hold" is in effect you will not be able to register. We hope you will be able to return to your studeis at Ohio University which you can do after medical clearance has been granted by a health care professional treating your medical condition. If you decide to apply for re-enrollment to Ohio University, you must complete the following four steps:
1. Write to Counseling and Psychological Services (CPS) concerning your re-enrollment at least one month prior to the first day of classes for the quarter you wish to attend.
2. Have the practitioner in charge of your treatment forward to us a completed Provider Report Form. You may obtain a Provider Report Form by calling CPS at 740-593-1616, or see below to download this form.
3. You will need to sign a form authorizing the release of information so that a representative from Counseling and Psychological Services can speak to your practitioner in charge of your treatment. 4. Please call CPS to verify that the Provider Report Form and Release of Information Form have been received and answer any remaining questions. The University retains the right to grant final clearance. Therefore, it may be necessary for CPS to conduct an evaluation interview prior to re-enrollment.
